Who We Are

CTC Disability Services (formerly CROSB) is part of South Burnett CTC Inc (CTC), a community organisation that provides a range of services and supports to families and individuals across the South Burnett region.

CTC Disability Services offers the benefit of over 5 years of experience in providing a variety of support and activities to individuals, their families and carers.

CTC Disability Services is currently accredited under the Human Service Quality Framework for the provision of the following in-scope Disability and NDIS Services:

Our Services

Plan Management

A participant chooses to use a plan manager provider who will assist a participant with managing their NDIS plan funds. Plan management providers can purchase supports on behalf of participants from either registered or unregistered providers. A plan manager will receive funds from the NDIA and disburse funds on behalf of a participant to providers of other services received.
Plan management funding is included in a participant’s plan, but is separate to the reasonable and necessary funding of a participant’s NDIS disability supports.

A participant can choose to change plan management providers at any time during the life of their NDIS plan, as long as a participant complies with the notice periods agreed in their service agreement.

A plan manager is responsible for processing payment requests through the NDIS myplace provider portal, or via other technology options available from the NDIA such as Application Programming Interfaces.

Plan management providers must follow payments and pricing controls established by the NDIA. This includes abiding by, and not exceeding, price controls set out in the NDIS Price Guide. Plan managers are unable to pay an invoice for a support that exceeds the relevant price control, irrespective of whether a provider seeking payment is a registered or non-registered provider. Plan managers cannot claim funds from a service booking in advance prior to supports and services being delivered to a participant. Plan managers can only claim from a participant’s NDIS plan once a support has been delivered or provided, and an invoice or receipt has been raised showing the relevant information about the type of service and when a service was provided to the participant.

A plan manager’s role is to support a participant by managing the funding of supports in their NDIS plan and providing a monthly NDIS Budget Report.  Their role relates specifically to the financial, plan-managed funded supports of the NDIS Plan and does not extend into supporting with day-to-day administration, management or maintenance of the entire NDIS plan. A plan manager has different roles and functions to the plan implementation support provided by an Local Area Coordinator, Early Childhood Partner, or Support Coordinator.

Support Coordination

My role as support Coordinator is help guide NDIS Participants to better understand how their NDIS funding can help them overcome their disability barriers. I draw upon my experience to link participants to the best possible support services, allied health therapists and community organisations to help them meet their NDIS Plan Goals.  The NDIS can be a complex, but using my knowledge and expertise I can assist participants and their carers to navigate all aspects of the scheme to ensure funds are being managed appropriately, mandatory reporting is completed, help design supports to build the participant’s  capacity and independence and act as the central contact point to ensure that people with a disability can improve their lives.

Frontline Service Delivery

At South Burnett CTC Disability, we’re all about offering top-notch support to people in need. Guided by the NDIS Quality and Safeguards Commission, registered with the NDIS, we put together personalised support plans tailored to each person’s unique needs. Our services include things like personal care and helping people get out and about in their communities. We believe in giving people choice and control, so they can build independence and get involved in their communities. We’re committed to inclusion and respect, ensuring everyone has the support and opportunities they need to thrive.

In-Home Services: Support to maintain independence at home, including accommodation assistance and flexible personal care.
Respite Support: Options for in-home or out-of-home care, available weekends with activities to support both individuals and carers.
Community Access & Participation: Assistance to engage in community life, access services, and join tailored recreational and social activities.
Group Nights: Monthly events for social connection and new experiences.
Learning and Life Skills: Development of essential skills like cooking, technology use, and financial management to enhance independence and confidence.
NDIS Registered Provider: Comprehensive supports to achieve personal goals, including core supports and capacity building.
Monthly Day Trips: Outings beyond South Burnett for activities not easily accessible locally.

Supported Independent Living

CTC Disability runs two Supported Independent Living (SIL) homes in the area: Nanango Heritage Lodge and Tessmans Road House. Heritage Lodge in Nanango offers a supportive setting with daily living help, personal care, medication management, and community access. Tessmans Road House in Kingaroy provides similar services, focusing on helping residents stay independent while offering the support they need. Both homes are dedicated to improving the quality of life for those seeking a good mix of independence and care.
