Service Integration Initiative

 

This program is part of a State wide initiative providing a backbone of support for housing and homelessness networks along with Place-Based Response Teams and the Service Integration Initiative, delivered in key locations across Queensland. This role will work across the South Burnett region

 

How We Help

  • Coordinated Support – We connect service providers through two key groups:
    • South Burnett Housing Integrated Response Group – A panel of local services working together to find solutions for people with complex housing needs.
    • South Burnett Housing and Homelessness Network – A group of organisations collaborating to identify gaps, improve referrals, and strengthen support for people at risk of homelessness.
  • Sustainable Housing Outcomes – We focus on long-term solutions by improving service coordination and strengthening partnerships.
  • Referrals and Collaboration – Service providers can refer people in need to the South Burnett Housing Integrated Response Group for tailored support.

  • Specialist homelessness services
  • Government and non-government organisations
  • Community groups and support services

  • Service providers – If your organisation wants to join the South Burnett Housing Integrated Response Group, complete our Expression of Interest form and we will be in touch.
  • Meetings – Our groups meet monthly. If you would like to be part of the South Burnett Housing and Homelessness Network, email lyndah@sbctc.com.au to join.

Immediate Housing Response
This program assists families through support and brokerage to access immediate accommodation and longer-term housing options.

Malcolm’s Youth Hostel
The Youth Hostel is located in Kingaroy and provides crisis accommodation for up to 6 young people aged 16-25.

Youth Outreach/Mobile Support
Our service supports young people who are homeless or at risk of homelessness to access or maintain their accommodation.
