Support Coordination
Navigating the NDIS can be tricky, but you don’t have to do it alone. Our Support Coordinators are here to guide you every step of the way, helping you understand how to best use your NDIS funding to break down barriers and reach your goals.
We connect you to the best supports, services, and allied health professionals to help you thrive. Whether it’s understanding your funding, building your capacity, or connecting with local services, we’re here to make sure you’re getting the most out of your NDIS plan. We’ll support you with the important steps of managing your plan and make sure you’re connected to the services and opportunities that fit your needs.
Our support coordinators are your central point of contact, helping you to navigate the NDIS with ease and ensuring that every step is taken to help you live the life you want.
Frontline Service Delivery
At South Burnett CTC Disability, we pride ourselves on providing personal, compassionate support to individuals in need. Registered with the NDIS and guided by the NDIS Quality and Safeguards Commission, we’re committed to offering tailored services that make a real difference in people’s lives.
Our Frontline Services include everything from personal care to helping you get out and about, and everything in between. Whether you need support in your home or help with community access, we’re here to provide the assistance you need to stay independent, get involved, and live the life you choose.
We believe in offering choice and control, allowing you to create a life that’s meaningful and connected. Whether it’s participating in social activities, improving life skills, or simply getting out into the community, our team is here to support you every step of the way.
Our services include:
In-Home Services: Support to maintain independence at home, including accommodation assistance and flexible personal care.
Respite Support: Options for in-home or out-of-home care with activities to support both individuals and carers.
Community Access & Participation: Assistance to engage in community life, access services, and join tailored recreational and social activities.
Group Nights: Monthly events for social connection and new experiences.
Learning and Life Skills: Development of essential skills like cooking, technology use, and financial management to enhance independence and confidence.
NDIS Registered Provider: Comprehensive supports to achieve personal goals, including core supports and capacity building.
Day Trips: Outings beyond South Burnett for activities not easily accessible locally.
Plan Management
At South Burnett CTC, we understand how important it is for you to have the support you need in managing your NDIS plan. With our Plan Management service, you’ll have a friendly, knowledgeable team behind you to help manage your NDIS funding, so you can focus on what truly matters—living your best life.
A Plan Manager works with you to manage your NDIS funds, helping you purchase supports from both registered and unregistered providers. We’ll ensure payments are made on time, and we’ll give you regular updates with monthly NDIS Budget Reports, so you always know where your funding is being allocated.
We’re here to make sure everything runs smoothly. If you ever want to change your plan manager, you can do so at any time by following the notice periods in your service agreement. Our plan managers will process payments through the NDIS myplace provider portal or other available technology options, ensuring everything is handled according to the NDIS rules and guidelines.

Supported Independent Living (SIL)
At South Burnett CTC, we offer Supported Independent Living (SIL) for those who need support but still want to live as independently as possible. We operate two SIL homes in the South Burnett: Nanango Heritage Lodge and Tessmans Road House in Kingaroy.
Both homes provide the perfect balance of support and independence. At Heritage Lodge, residents enjoy help with daily living tasks, personal care, medication management, and access to the community. Tessmans Road House focuses on helping residents maintain independence while offering the support they need to live comfortably.
Our SIL homes are dedicated to enhancing the quality of life for individuals seeking a mix of independence and the care they need to thrive.
